GUILLAIN – BARRÉ  SYNDROME(GBS)

 

Guillain-Barre Syndrome(GBS) , is an acute, immune-mediated disorder of peripheral nerves, spinal roots, and cranial nerves, commonly presenting as a rapidly progressive, are flexive, relatively symmetric ascending weakness of the limb, truncal, respiratory, pharyngeal, and facial musculature, with variable sensory and autonomic dysfunction.

 

An interesting fact is that, recent reports reveal that U.S. President, Franklin D. Roosevelt was not paralyzed because of polio, but by GBS.The reported incidence rates for GBS are 1–2 per 100,000 populations. The lifetime likelihood of any individual acquiring GBS is 1:1000.

 

The exact etiology of GBS is unknown. In GBS, the myelin sheath becomes damaged, causing nerve damage, resulting in faulty sending of signals between nerves and muscles. This faulty wiring causes muscle weakness, numbness and tingling, and eventually paralysis. There is more commonly symmetrical weakness in the lower limbs initially, which rapidly progress in an ascending fashion. Surgery, childbirth or vaccinations may also trigger the syndrome. Symptoms include pain in spine and limbs,instability, dysesthesias, numbness or tingling, muscle weakness, which gets progressively worse,respiratory problems etc. Patients often complain of rubbery legs reflexes, weakness and tingling sensation in legs,severe pain in lower back possibly spreading to upper body and arms, weakness in breathing,and in extreme cases, paralysis. Diagnosis includes exam, electromyography nerve conduction velocity test,spinal tap (lumbar puncture) and blood tests.
In treatment for GBS,not all treatments and therapies may be indicated. Intravenous immunoglobulin (IVIg)-at a starting dose of 2 grams per kg of patient weight, may be given in 5 divided dosages over 5 days.  Concentrated antibodies taken from healthy donors can be administered as they can block and destroy the harmful antibodies that are attacking the nerves. Plasmapheresis aims to remove the cells that are attacking the nerves. Pain medications such as naproxen, Ibuprofen, aspirin, acetaminophen, tramadol, pentazocine etc are given to relieve the patients from severe pain. Physical therapy, speech therapy, supportive therapy and ventilator services are provided to the patients if needed. Safety measures should be taken for compensating the loss of sensation.

Vitamin D Pills: An Objection



Vitamin D is a key component for healthy bones, teeth and muscles. It is produced naturally when the skin is exposed to sunlight or derived from foods such as oily fish, egg yolk and cheese. Previous research had shown significant effect of Vitamin D on preventing heart attack, stroke, cancer or bone fractures.

But the new study by Mark Bolland, University of Auckland - New Zealand, published in The Lancet Diabetes and Endocrinology strengthens arguments that Vitamin D deficiency is usually the result of ill health, not the cause of it.
There are some similar studies: In March 2013, British scientists, in a comparison of 4,000 women, found that vitamin D supplements taken in pregnancy made no difference to the child's bone health. And in September 2012, researchers at New York's Rockefeller University saw no evidence that vitamin D supplements lowered cholesterol, a factor in heart disease, at least over the short term.
These studies conclude there is “little justification” for physicians to prescribe Vitamin D supplements as a preventive measure for heart attack, stroke, cancer or bone fractures.

Alshifa Hospital Free Medical camp: Leap To Good Health
The Al Shifa hospital Pvt.Ltd which is always committed in providing state of art treatment to poor and needy at affordable cost conducted two free medical camps in February and March of 2014 in conjunction with two different organizations. The medical care of various sections of society is important for balance in society and here lies the importance of role of non-government organisations where government authorities cannot reach due to their limited resources.
Medical camp: Along with KVVES
The Mannarkad Branch of Kerala vyaparavyavasayiEkopanasamithi(KVVES), one of the largest single organisations of traders and industrialists in the world organised a free medical camp at VyaparaBhavan, Mannarkad on 1st march 2014. The camp was organised with the objective of providing free medical examination and check up for the traders and their family members. Dr.Omnath MD,DM (cardiology) headed the camp along with Dr.Reghuram.A.krishnan MD, DM,FIC(cardiology) and Dr.Abeer MBBS,MEM (GWUU) USA.Various free services were made available in the camp such as general check-up, cardiac check-up, medicine distribution, ECG and blood test. One of the important features of the camp was that besides active medical treatment it also provided an awareness programme on First Aid and its importance which is a basic treatment to patient and can be lifesaving in certain occasions.
Medical Camp: Along with BijuRafel RemembranceCommittee
More than 200 people belonging to all walks of life thronged A.M.L.P. school,Kottapuram, Angadipuram on 16th February 2014 for a special medical camp Organised by Biju Rafel Rememberance committee in memory of BijuRafel activist. The camp was performed in collaboration with Al Shifa hospital and emphasised on eye check-up and cardiac check-up. The cardiologist Dr.omnath MD, DM,Dr.Reghuram.A.krishnan MD, DM, FICand eye specialist Dr.PadhmanabhanMeleth (ophthalmology) contributed much in thediagnosis and treatment. In the eye check-up they identified few patients who require further help. Blood donation campaign was also conducted as part of the camp. Free medicines and free medical test were also provided where ever necessary.
Both medical camps were benefited over hundreds of people across state. The beneficiaries and local people where happy with the conduction of these medical camps and are satisfied with doctors and their services. The people of the area ensured their fullest cooperation and help with Al Shifa hospital for developmental and relief activities. Each camp signified the selection of different location for future camps where there is greater need.
